IS223 Student Digital Learning Device Request
Families, please complete this form to request to borrow a digital learning device from school. These devices are issued to students by the NYC Department of Education and are due back to the school when the student graduates or transfers to another school.

PLEASE READ BEFORE COMPLETING THIS FORM:
  • If you already have a DOE-issued or school-issued device that is physically damaged or not working, you must return it to Mr. Raubvogel in room 202 before requesting a new one.
  • If you lost your device or it was stolen, you must report it to info@is223montauk.org – we will assist you in filing a police report to obtain a replacement device.
  • If your child transfers to another school, graduates, or no longer needs their device, you must return it to Mr. Raubvogel in room 202.
  • As of September, 2023 – inventory is limited and requests will be filled on a first-come, first-served basis. If a device is not available for you, we will hold your place on a waiting list.
  • New devices are now WiFi-only – inventory of devices with an LTE-mobile data connection is extremely limited due to funding.
